How to Register
Firstly, thank you for your interest in Albany House Surgery! To register with us please call and speak to one of our friendly Reception Staff on 01905 26086. We will guide you through the process. Below is a link to the Albany Registration Forms. When you register, please ask for information about Online Services. Having an online account would allow you to book appointments and request prescriptions at a time to suit you. More features are coming soon.
Patient Preference for Practitioner
A patient has the right to express a preference as to which doctor they would like to see. Please tell the receptionist which doctor you would like to see. Urgent cases will be seen on the day but not necessarily with the doctor of your choice.
Please only ask for an emergency appointment when you have a problem that cannot wait.
Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
We have the right to refuse to register patients if they have a violent flag against their record.

Download a form to opt out of having a Summary Care Record (SCR) or opt out of additional information being included in the SCR.
Organ Donation
From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
